Three forms are required to become a new patient / client. Before completing the forms it may be helpful to speak with me by phone, just to make sure that I am familiar with what you want to accomplish, and that I am taking new patients.

Complete one of these questionnaires. Choose the form that applies. If I will be seeing you as a couple, each person should complete an adult questionnaire.
